The Sacraments are "efficacious signs of grace, instituted by Christ and entrusted to the Church, by which divine life is dispensed to us." 

This means that Jesus desired to continue to nourish his people through physical signs that actual confer grace to each individual. In short, the sacraments help us to encounter Christ, be filled with his presence and carry out his command to make disciples of all nations.
